PATHWAYS…An Innovative Approach to Rehabilitation
The unfortunate effects of an injury or illness can leave one weak, dependent and with a variety of limitations…as well as questions: “What can I expect? Where do I go from here? What will the future be like?” We understand that you want and need answers, the best possible treatment plan and ultimately, the ability to live as independently as possible. In response, Vienna Square has developed a specialized rehabilitation program called “Pathways”.
Pathways is a program consisting of Physical Occupational Therapists and Speech/Language Pathologists working with nurses, aides and your physician as a cohesive team to plan a rehab path with the singular goal of helping you reach your maximum potential as quickly as possible.
Our therapy program was built from the ground up, rethinking every assumption and striving to provide the most advanced programming available. Systems were designed specifically to support all aspects of practice, from minimal assist training to the acute rehab needs. Our gym is a place where expert therapists are empowered by advanced tools and the best practices to create a specific therapy pathway for each individual.

Many of our skilled patients will come to us for the first time from a private home, a hospital or another healthcare facility. Consequently, our Pathways program provides short-term rehab care as an interim step prior to returning home.
However, since we are a Continuing Care Campus, patients will also come to us from our independent and assisted living communities, as it is not uncommon for someone to move among levels of service within our community. Whatever the circumstance, our care and lifestyle are based on individual need with respect for privacy and dignity.
